On February 5, 2016, McIntosh was found to have used excessive force against inmate Michael Holloway by grabbing his shirt, leading him down a corridor, and shoving him into a door at the Middletown City Jail. This incident marked his 29th reported use of force in three and a half years, more than double the incidents involving all other corrections officers combined. McIntosh received a one-day suspension without pay, which he covered using holiday pay, and resigned less than three months later.
