In 2004, Officer David Mahoney was charged with conduct unbecoming an officer for engaging in an inappropriate sexual relationship with a 17-year-old female cadet from the Warwick Police Explorer Program. The cadet alleged that they had consensual sexual relations on five occasions between April and August 2003. A Hearing Committee recommended his termination from the Warwick Police Department.
