In May 2009, Officer Perez engaged in a high-speed chase without activating his lights and siren while speeding, and failed to activate his siren when passing through a red light. This incident, along with prior disciplinary actions—including an April 2008 suspension for engaging in a vehicle pursuit against department policy, a July 2008 demotion for lack of veracity during a supervisor inquiry, and a May 2009 verbal admonition for excessive speed—led to his termination in November 2009. The Utah Court of Appeals upheld his termination in 2014. ([archive.sltrib.com](https://archive.sltrib.com/article.php?id=57506577&itype=cmsid&utm_source=openai))
