    On June 2, 2011, Officer Phillips responded to an emergency call by driving at speeds up to 121 mph through multiple intersections without proper regard for safety, violating department policies. This incident was part of a pattern of poor judgment and policy violations, including a March 2011 counseling letter for failing to handle a companion’s vulgar comments appropriately, a September 2010 verbal warning for missing court appearances, a five-day suspension in March 2010 for a verbal and physical altercation, a verbal warning in September 2009 for using a taser on an individual upon request, and a suspension with remedial training in October 2008 after a traffic accident. Phillips was terminated on June 30, 2011, and his termination was upheld by the Utah Court of Appeals in 2013.
